New and Prospective Patients Welcomed
On your first visit you will need the following:
Arrive 15 minutes early to fill out necessary paperwork.
Your insurance card.
Co-pay/Co-Insurance if applicable. We accept checks, cash and credit cards.
Your doctor’s referral/prescription.
Please wear comfortable loose clothing for the therapist to perform the evaluation.
You will want to schedule out ahead 3-4 weeks to obtain your desired treatment times. Please
be aware that the therapist’s schedules tend to get quite full.
Parents, please do your best to have childcare arranged for you children. The physical therapy
environment is not safe for children to be out in the gym.
