New Patient Information

What should I expect on my first visit?

You will be scheduled with the therapist who is best suited to treat your specific ailment. While all of our therapists are highly skilled and educated, each therapist has different areas of expertise and can be read more about here.  Your initial visit will be your evaluation and will take a bit longer than your other appointments. During this time you and your therapist will formulate an individualized plan of care.  After your evaluation, your appointments will consist of manual therapy, education, various modalities, and therapeutic exercise. We also offer aquatic therapy.

We ask that you bring the following information:

  • Insurance Card
  • Photo Identification
  • Physician referral or prescription
  • Applicable co-pay
  • L&I claim form

Helpful tips for your first visit:

  • Please arrive 15 minutes early to your appointment to allow time to complete your paperwork.
  • Wear comfortable clothing that allows you to move freely and provides clear access to your injured body part. We have full men and women’s locker rooms to secure your belongings if you need to change.
  • Our office is located inside of the Redmond Medical Center on the ground floor. There is a free parking lot with plenty of spaces.  The building is across the street from the Redmond Transit Center and the skate park.
  • While we do verify insurance coverage as a courtesy, it is recommended that you also contact your insurance company to understand your physical therapy benefits and deductible/coinsurance responsibilities.

Not every insurance we accept is listed here. If you do not see your insurance, please call our office to confirm we are in-network.

 

How do I schedule my first appointment?

If it is an insurance requirement, please have the prescription or referral for physical therapy from your doctor prior to making an appointment.  Insurance companies generally accept physical therapy referrals from physicians, naturopathic doctors, chiropractors, or PA-Cs/ARNPs. You may also utilize Direct Access.

We will ask you some important questions regarding your insurance coverage and demographics.  Please have your insurance card in front of you so that you can provide us with your member or subscriber ID and group number.

Please allow an hour and a half for the initial evaluation visit and an hour for subsequent visits.  We request 24 hrs notice for appointment cancellations.
